Medicaid Home and Community-Based Services, Including Paying a Family Caregiver

What this program is, in plain English

Medicaid was built to pay for nursing homes. Home and community-based services, usually called HCBS waivers, let that same money follow you home instead: personal care, help with bathing and dressing, homemaker services, adult day programs, respite so a caregiver can rest, home modifications like a walk-in shower or a ramp, and emergency response systems. The part families almost never hear is this. Most states run a self-directed option, sometimes called consumer-directed care, participant-directed care, or Cash and Counseling. Under self-direction, you are the one who chooses the caregiver, and in most states that caregiver can be your own adult child, your grandchild, your sibling, your neighbor, or your friend. They are hired through a financial management agency, they are paid an hourly wage, and the work they are already doing for free becomes paid work. A spouse can be paid in some states and not in others. Every state runs this differently and every state has a different name for it, so the question to ask your Area Agency on Aging or state Medicaid office is exactly this: does our state have a self-directed personal care option, and can a family member be the paid caregiver? Many waivers have waiting lists, so ask to be screened today even if the wait is long.

Who qualifies

  • Income limit: The federal rule is 300% of the SSI payment rate — about $2,982 a month for one person, or $4,473 a month for a couple, using the 2026 rate.   • Countable asset limits for long-term-care Medicaid are set by each state and are commonly around $2,000 for one person, with separate protections for a spouse who stays at home. We do not publish a figure for your state unless we could read it off that state's own page — call your state Medicaid agency to confirm.
  • Requires both financial eligibility for Medicaid long-term care, which uses different and more generous rules than regular Medicaid, and a functional assessment showing you need a nursing-home level of care. Assets are counted, but a spouse who stays in the community is protected.

How to apply

This program is run by State Medicaid agency, often through your Area Agency on Aging. Most decisions take 45 to 90 days for financial eligibility, plus a separate functional assessment. Waiver waiting lists in some states run years..
